General Description
Support people in their home with intellectual and developmental disabilities (I/DD) to achieve maximum independence integrating agency core values. Provide reliable, compassionate and consistent care while creating meaningful, quality-based relationships in a supportive, cultural environment.

Essential Functions
- Monitor, assess and advise the management team on health observances and needs, while promoting health and wellness through nursing processes.
- Establish a positive and open communication style with everyone supported including all employees, family members and other health care providers.
- Promote wellness by applying nursing processes.
- Outline all monthly appointments; provide documentation and information needed to successfully complete the appointment.
- When indicated clarify hospital discharge, medication orders, and/or plans of care.
- Participate in hospital discharge or intake process, along with Director of Nursing.
- Provide instructions and education to those supported and staff as it relates to medical diagnoses and healthcare needs (i.e. Allergies, medications, diet, chronic and acute plans of care, etc.)
- Coordinate care with clinics, outside physicians, and hospitals. Attend appointments, as needed.
- Experience with G-tubes, J-tubes, Trach care.
- Other job duties as assigned.
Documentation
Assist with maintaining medical records for all people supported.
Update EMAR. Check and report medication errors, omissions or other concerns to management/RN.
Supervise the disposal of discontinued medications (and/or other medical supplies) in accordance with legal requirements.
Training
Provide training to support staff on people's needs including: competencies, medical history, treatments and wellness plans.
Monitor staff progress and assess any need for additional training. Report observations to the management team.
Participate in required state and agency trainings, including staff meetings.
Provide guidance and support to staff designated to order and stock medications.
Additional Functions of the Job
Assist with providing coverage for vacancies and peers during vacations.
Assumes responsibility for ongoing professional development by participating in community-based educational trainings.
All other duties as assigned by supervisor(s).
Certification, Licenses, Registrations
- Graduate from an accredited school of nursing -
RN: Associates or Bachelor's of Science in Nursing.
LPN: Licensed Practical Nurse degree.
- Experience providing care for chronic, acute and mental health needs.
- Must possess a valid Rhode Island Nursing license.

Schedule & Benefits
Full Time: 36 hours (4 days off )
3 Shifts: Wed/Thurs/Sat 7pm to 7 am
Pay Rate: RN $40.50 hourly
LPN $37.00 hourly
