About the Role
As an Intake Representative in Supportive Care, you will be responsible for receiving and processing referrals for home care and hospice patients, ensuring accurate data collection and entry. You will verify physician orders, patient demographics, and insurance status while collaborating with Care Support to address Social Determinants of Health needs. This role is essential in maintaining smooth operations and providing excellent customer service to patients, families, and healthcare partners.
What We're Looking For
Education: High school diploma or equivalent.
Experience: At least one year in a home healthcare environment.
Skills & Abilities:
Proven ability to deliver exceptional customer service.
Knowledge of third-party payer coverage verification.
Strong problem-solving skills with the ability to adapt and resolve issues independently.
Excellent organizational skills to coordinate multiple tasks and maintain workflow.
Proficiency in Microsoft Word and ability to operate standard office equipment.
Strong written and verbal communication skills.
What You'll Do
Receive and process referrals for Home Health, DME, and Hospice services.
Assist patients, families, physicians, and referral sources with inquiries and support.
Ensure accuracy and integrity of patient data.
Prepare necessary correspondence and materials for external agencies.
Contact third-party payers for insurance verification and benefit precertification.
Maintain up-to-date records of home health and hospice agencies across multiple states.
Communicate with agencies regarding referrals, orders, and other coordination needs.
Provide resources and support for patients experiencing Social Determinants of Health challenges.
Document patient and family interactions in the EMR system.
Educate patients on accessing the patient portal and assist with financial and insurance forms as needed.
Perform additional duties as assigned.
Physical Requirements
Light physical work, including occasional lifting of up to 25 lbs. and frequent lifting of up to 10 lbs.
Activities may include standing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
Good balance, coordination, and visual acuity required for data entry and computer work.
Work is performed in a general office environment with minimal exposure to adverse conditions.
