As a WCBH Access Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a caseload of clients receiving community based and in office skills-based therapy. The primary role of the Access Care Coordinator will be to provide resources, brief interventions, case management and follow clients throughout the Intake process until assigned to a provider.
Essential Duties and Responsibilities:
- Assesses patient needs and goals, provides brief intervention services, makes referrals, links to community resources, assists with transitions of care, monitors and follow-up.
- Facilitates care coordination/communication with both internal (i.e., treatment team) and external client stakeholders.
- Participate in the development and implementation of treatment plans, case management plans and other clinical documentation
- Promote effective utilization of clinical resources, community resources and other social supports
- Assist clients connect with resources by sharing information and applying for benefits, when necessary
- Care coordination/communication both internally and externally to facilitate continuity of services

Education:
- Associate’s degree in Social Work, Rehabilitation, Psychology, Education, or Human Services related field with 2 years work with person(s) who have severe mental health disability or understanding of mental illness acquired through provision of support to family members with mental illness.
- Bachelor's degree in a relevant field
Experience:
- 2 years’ related experience
Required Skills:
- Excellent communication, problem-solving, time management, and organizational skills required
- Must be flexible and demonstrate the ability to work with minimal supervision
- Excellent computer skills and knowledge of Microsoft Office
- Ability to learn necessary computer and technical skills essential to the position and Agency needs
- Ability to relate to staff, clients and community around a mental health system
- Maintain skill competencies in customer service, quality improvement, compliance and safety
Preferred Skills:
- Knowledge of and experience with electronic medical record system, preferred.
- Experience providing targeted case management services
- Prior experience in a community mental health environment
- Knowledge of evidence based mental health treatments such as Dialectic Behavioral Therapy, Cognitive Behavioral Therapy, Enhanced Illness Management and Recovery, and Prolonged Exposure for PTSD
