As a WCBH Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a caseload of clients receiving community based and in office skills-based therapy. The primary role of the Care Coordinator will be to assess needs, coordinate care and connect individuals with community resources. The Care Coordinator will also support clinical processes associated with mental health treatment when applicable.
Essential Duties and Responsibilities:
- Help clients and families understand treatment options and make informed decisions through regular in office and phone check ins.
- Work with treatment team to facilitate coordination of care and information sharing
- Participate in the development and implementation of treatment plans, case management, and other clinical documentation
- Promote effective utilization of clinical resources, community resources and other social supports.
- Assist clients with resources by sharing information and applying for benefits, when necessary.
- Care coordination/communication both internally and externally to facilitate continuity of services.
