Position Summary: The Community Health Worker (CHW) works primarily with the CalAIM program under supervision, providing direct services to patients, building trusted relationships with the individuals they serve to improve health and well-being over time.
Specifically, the CHW will work within WCHC's Healthcare for the Homeless team to improve evidence-based medical and behavioral health integration practices within primary care, specialty care, and behavioral health.
Responsibilities:
- Provides health coaching, patient navigation, health education and/or health promotion for a diverse patient panel within assigned health program.
- Conduct extensive outreach activities within a specific health program and/or to defined patient populations.
- Assists Primary Care Team with identification of supportive services and resources available to their patients to meet specific needs.
- Connects patients with supportive services and resources that are critical to their overall health and wellness.
- Works with Primary Care Team to provide Care Management and Care Coordination including engaging families, providing home visits, addressing basic needs, health coaching.
- Works with Primary Care Team to assist with Care Management for Complex Care Patients including supporting hospital transition, barrier reduction, health coaching, and system coordination.
- Works collaboratively with Care Team to optimize communication, coordinate needed services, and meet the needs of the patient.
- Participates in activities related to addressing the social determinants of patient and community health such as assessing community strengths, advocating for needs of individuals and families, and assisting with community organizing.
- Conduct daily data entry within the eCW system to track patient encounters and progress.
Skills and Experience:
- Certificate of completion from Community Health Worker Program or equivalent experience.
- 1-2 years’ experience working with unhoused / homeless patient population or within project area, ideally within a healthcare or community setting.
- Demonstrated commitment and skills to provide appropriate care to a diverse population.
- Knowledge of community resources and networking ability
- Basic computer skills including commonly used applications such as Outlook and Word.
- Must have an acute attention to detail and follow through on commitments.
- Must have ability to manage priorities and workflow.
- Expected to have strong interpersonal skills and can understand and follow written/verbal instructions.
- Ability to work independently with little or no supervision.
- Must have a commitment to excellence and high standards.
- Ability to demonstrate a positive customer service, patient centered approach always.
Physical Demands:
- Ability to move freely about the clinic/office and carry light loads (up to 25 lbs.)
- Ability to sit for extended periods of time and stand for minimum periods of one hour.
- Ability to interface with computer monitor for extended periods of time.
