Key Responsibilities
- Knowledge of transition legislation and the ability to educate teachers, families and students the rights and responsibilities identified in the law.
- Develop, implement, and monitor individualized transition plans for students with disabilities in collaboration with the IEP team.
- Collaborate with case managers and counselors to ensure transition goals and objectives are being planned and met.
- Conduct transition assessments to identify students' strengths, interests, and areas for growth.
- Teach in a structured classroom career related skills to define goals and seek and maintain employment.
- Provide guidance to students on post-secondary options, including college, vocational training, and employment opportunities.
- Facilitate connections with community agencies (DDS, Autism Waiver, BRS-Level up, Workforce Partners, Department of Labor) and resources that support transition planning.
- Facilitate connections with local businesses and business leaders
- Organize and facilitate information on transition skills, programs, agency information and programming with parents.
- Assist students in developing self-advocacy, self-determination, and independent living skills.
- Collaborate with teachers, counselors, businesses and families to ensure transition goals are integrated into students' educational programs.
- Organize and deliver workshops, training, and resources for students, parents, and staff on transition-related topics.
- Maintain accurate records of student progress, assessments, and transition services.
- Attend and participate in CSDE and District professional learning
- Experience working with students with disabilities, particularly in transition planning.
- Strong communication, organization, and collaboration skills.
- Knowledge of community resources, area businesses , vocational services, and post-secondary options for students with disabilities.
- Ability to build positive relationships with students, families, and community partners.
- Experience developing and implementing transition assessments and writing measurable IEP goals and objectives.
- Familiarity with assistive technology and accommodations that support student success.
- School-based with occasional off-site meetings or visits to community agencies,colleges, and workplaces.
- May require occasional evening meetings or workshops for parents and families.
