West Hartford Public Schools seeks to fill the position of Transition Coordinator at both Conard and Hall High School. The Transition Coordinator plays a vital role in supporting students with disabilities as they prepare to transition from high school to post-secondary education, employment, and independent living. This position involves collaborating with students, families, school staff, and community partners to develop individualized transition plans that align with each student's goals and abilities.

Requirement: Current CT Certification (165).

Key Responsibilities

  • Knowledge of transition legislation and the ability to educate teachers, families and students the rights and responsibilities identified in the law.
  • Develop, implement, and monitor individualized transition plans for students with disabilities in collaboration with the IEP team.
  • Collaborate with case managers and counselors to ensure transition goals and objectives are being planned and met.
  • Conduct transition assessments to identify students' strengths, interests, and areas for growth.
  • Teach in a structured classroom career related skills to define goals and seek and maintain employment.
  • Provide guidance to students on post-secondary options, including college, vocational training, and employment opportunities.
  • Facilitate connections with community agencies (DDS, Autism Waiver, BRS-Level up, Workforce Partners, Department of Labor) and resources that support transition planning.
  • Facilitate connections with local businesses and business leaders
  • Organize and facilitate information on transition skills, programs, agency information and programming with parents.
  • Assist students in developing self-advocacy, self-determination, and independent living skills.
  • Collaborate with teachers, counselors, businesses and families to ensure transition goals are integrated into students' educational programs.
  • Organize and deliver workshops, training, and resources for students, parents, and staff on transition-related topics.
  • Maintain accurate records of student progress, assessments, and transition services.
  • Attend and participate in CSDE and District professional learning

Qualifications

  • Experience working with students with disabilities, particularly in transition planning.
  • Strong communication, organization, and collaboration skills.
  • Knowledge of community resources, area businesses , vocational services, and post-secondary options for students with disabilities.

Preferred Skills

  • Ability to build positive relationships with students, families, and community partners.
  • Experience developing and implementing transition assessments and writing measurable IEP goals and objectives.
  • Familiarity with assistive technology and accommodations that support student success.

Work Environment

  • School-based with occasional off-site meetings or visits to community agencies,colleges, and workplaces.
  • May require occasional evening meetings or workshops for parents and families.
