What are the responsibilities and job description for the Case Manager position at West Houston Assistance Ministries - WHAM?
The Case Manager is responsible for working with clients who are impacted by any of the following; COVID-19 pandemic, homelessness and exposure to domestic violence, etc. This position requires a demonstrated ability and experience working effectively with clients in crisis. The Case Manager is responsible for creating a well-developed income-based case plan for clients through employment or social service linkages that address their barriers to securing and maintaining housing. This position is responsible for assisting clients who are at risk of becoming homeless and works to complete comprehensive assessments through the use of the Wrap Service Model that determines the nature and degree of assistance for which the client qualifies.
Education/Experience
High School Diploma or equivalent. Bachelor's degree in social work-related field. 1-5 years relevant work experience in place of a degree. Knowledge of community resources in the Houston area, including the Harris County Hospital District and MHMRA systems, is preferred. Experience working with a multi-disciplinary team is preferred. Bilingual (English/Spanish) preferred.

Ability to gather data, compile information, and prepare reports. Ability to monitor, assess and record client progress, and adjust plans and services. Ability to assess mental status in clients and to develop individual goals and plans. Excellent writing and verbal communication skills. Effective use of time management. Excellent technology skills and ability to use various platforms to connect with clients. Good organizational, time management, and cross-cultural communication skills. Ability to work with clients with mental health issues, disabilities, survivors of domestic violence, sexual assault, and other violent and traumatic crimes, and individuals in crisis.
Must also have a dependable automobile with a valid driver’s license and current insurance.
