About the Role
The Social Care Manager (SCM) is a leadership role responsible for strategic oversight of social care services in the areas of revenue cycle, care management, network development, and patient access. The SCM leads a team of Social Care Specialists to deliver high-quality care coordination, optimize workflows, and build strong relationships with stakeholders to advance health equity and patient access.
With expertise in patient access management, healthcare systems, care management, and revenue cycle operations, the SCM ensures seamless integration of care management programs, financial systems, and technology solutions for diverse populations.
Responsibilities
Team Leadership & Operations
- Lead and support a team of Social Care Specialists, fostering a collaborative, high-performance culture.
- Oversee day-to-day operations, ensuring alignment with care management goals, patient access initiatives, and revenue cycle performance.
- Provide coaching and training to support professional development and team growth.
Care Management & Strategy
- Develop and oversee care coordination workflows to align with FQHC standards, SDoH interventions, and patient navigation services.
- Collaborate with leadership to enhance social service delivery, improve patient access, and support care management strategies.
- Build and maintain partnerships with hospital leadership, health plans, and community providers to promote integrated care.
Revenue Cycle & Patient Access Integration
- Oversee patient access workflows, including registration, scheduling, and eligibility verification, to ensure efficient and seamless operations.
- Provide financial advocacy by assisting patients in navigating insurance coverage, understanding reimbursement processes, and addressing financial responsibilities.
- Implement and maintain effective workflows that optimize resource allocation, improve key revenue cycle metrics (e.g., AR days, cash collections, and denials), and ensure compliance with healthcare access standards.
- Monitor and enhance revenue cycle performance, integrating best practices to align with regulatory and organizational goals.
Quality Assurance & Performance Monitoring
- Ensure accurate documentation and compliance with care management and patient access standards.
- Use data-driven insights to evaluate team performance and identify opportunities for operational improvements.
Technology Integration
- Leverage tools like Epic systems, Resolute, Prelude, Cadence, and MyChart to enhance care delivery and operational efficiency.
- Train staff to effectively use healthcare and revenue cycle technologies.
Qualifications
Education and Certifications
- Bachelor’s degree in Public Health, Healthcare Administration, or related field required.
- Master’s degree or equivalent work experience preferred.
- Certified Healthcare Access Manager (CHAM) or similar qualification.
Experience
- 6 years of experience in healthcare operations, patient access management, revenue cycle management, or care management.
- 3 years in a leadership role managing cross-functional teams.
- Experience in FQHCs, hospital systems, and regulatory compliance is highly valued.
Skills
- Expertise in care management and patient access workflows, including registration, eligibility verification, and scheduling.
- Strong knowledge of revenue cycle operations, including insurance verification, billing, and denials management.
- Analytical skills to track and improve performance metrics.
- Proficiency in Epic systems and other revenue cycle tools.
- Demonstrated ability to foster relationships across diverse stakeholders and align strategies with organizational goals.
