About the RoleWe are seeking a dedicated Team Lead Supervisor to support the Social Care team in providing high-quality case management and care coordination services. This role will oversee and guide a team of Social Care Specialists to ensure effective service delivery, operational efficiency, and professional development. The ideal candidate will have strong leadership skills, deep knowledge of case management, and experience working with Medicaid and other government benefits.
Qualifications:Bilingual: Spanish is required, with fluency or advanced-level language skills.
Education: Bachelor’s degree in Human Services or a related field, or equivalent work experience. A Master’s degree is preferred.
Experience:
Minimum of 4 years of experience in care planning, care coordination, or case management.
At least 1 year of experience in a leadership, supervisory, or team lead role preferred.
Demonstrated expertise in government benefits applications, housing support, and managing complex cases.
Strong leadership and coaching skills to mentor Social Care Specialists effectively.
Excellent judgment and decision-making skills to advocate for clients’ best interests.
Proven ability to work collaboratively with diverse individuals and groups.
Comfortable conducting outreach and cold calls in medical or community settings.
Exceptional interpersonal and communication skills with the ability to build rapport with colleagues, clients, and stakeholders.
Must maintain current licenses and certifications required for the position, including (but not limited to) First Aid and CPR.
Ability to pass pre-employment and random drug screenings while maintaining professionalism, emotional control, and composure.
Reliable access to a vehicle, proof of insurance, and a valid driver’s license for essential job-related travel.
Proficient in computer applications, including electronic health record (EHR) systems.
Strong organizational skills and ability to manage multiple priorities effectively.
Supervision & Team Leadership
Provide oversight, guidance, and mentorship to Social Care Specialists to ensure high performance and compliance with organizational standards.
Assist with onboarding and training new Social Care Specialists, ensuring they understand policies, procedures, and service expectations.
Conduct regular check-ins and coaching sessions with team members to support professional growth.
Monitor workload distribution and support Social Care Specialists in managing caseloads effectively.
Address escalated cases and provide solutions to resolve complex client issues.
Case Management & Service Delivery
Manage a caseload of complex cases, ensuring thorough documentation in EHR systems and timely follow-ups.
Support clients by facilitating access to community resources and coordinating service referrals.
Conduct pre-screening for client eligibility and assist with applications for government benefits and services.
Ensure warm handoffs between service providers to promote seamless care transitions.
Operational Support & Compliance
Collaborate with leadership to enhance program outreach and community engagement.
Ensure accurate and timely documentation of client interactions, case notes, and referrals.
Track team performance metrics and report key insights to leadership.
Participate in internal meetings and provide updates on team progress and challenges.
Maintain up-to-date knowledge of policy changes, program procedures, and benefits to serve as a resource for the team.
Prepare and submit required reports, ensuring compliance with all organizational and regulatory standards.
Other Duties
Engage in professional development activities to enhance leadership and case management skills.
Support special projects and initiatives as assigned by leadership.
