Job Title: Health-Related Social Needs (HRSN) Screener

Department: Food Access Programs

Reports to: Director of Operations

Supervises: N/A

Status: Full-time,Non-Exempt Hourly

Salary Range: $18-$22/hr

Position Summary:

We are seeking a dedicated and detail-oriented Health-Related Social Needs (HRSN) Screener to join our team. The ideal candidate will be responsible for screening members for Health-Related Social Needs using the Accountable Health Communities (AHC) HRSN Screening Tool, ensuring compliance with all relevant processes and standards. This role involves direct interaction with community members, obtaining necessary consents, maintaining accurate records, and utilizing a specialized IT platform for data entry and reporting. The Screener will contribute to efforts to advance health outcomes and reduce health disparities by collecting critical data for health interventions.

ESSENTIAL FUNCTIONS

Screening:

  • Screen members using the AHC HRSN Screening Tool to collect information on consent, demographics, and social needs data.
  • Ensure informed consent is obtained from members before beginning screenings.
  • Minimize duplication of screenings by reviewing member screening history.
  • Ensure screenings are reimbursable by adhering to specific eligibility criteria (e.g., first screening of the year or major life event triggers).
  • Use the FindHelp platform to conduct screenings and submit data.

Data Management & Reporting:

  • Enter screening data accurately, ensuring compliance with reporting standards.
  • Collaborate with the WNY Integrated Care Collaborative on performance reporting and data collection for regulatory compliance and performance evaluations.
  • Ensure timely submission of screening data.

Training & Compliance:

  • Maintain cultural and linguistic competency in screenings, following Culturally and Linguistically Appropriate Services (CLAS) Standards and completing relevant training.
  • Ensure adherence to HIPAA and other healthcare regulations.
  • Complete mandatory training on child abuse and neglect reporting (as per New York State requirements).
  • Participate in other professional development and maintain certifications required by WSCS, the Western New York Integrated Care Collaborative, and any other relevant entities.

Technological Proficiency:

  • Utilize the FindHelp platform for conducting screenings, collecting data, and reporting.
  • Adapt to the use of new screening platforms and data reporting tools as necessary.
  • Work with live language interpreters using tools such as Language Line, Tarjimly, or Lingolet.

Languages & Accessibility:

  • Conduct screenings in multiple formats including in-person, telephone, virtual, and/or text messaging.
  • Conduct screenings in multiple languages with the assistance of live interpreters via phone call, app, and video tools.

Outreach:

  • Establish additional locations for conducting screening by partnering with healthcare providers, schools, and other organizations.

Other Duties as Assigned

Qualifications:

  • Associate or bachelor’s degree in social work, public health, or a related field or equivalent job experience.
  • Candidate must have a valid driver’s license, a clean driving record, and consistent daily access to a vehicle, as position requires outreach to sites around Western New York (mileage reimbursement provided for local travel).
  • Strong attention to detail and organizational skills.
  • Proficiency in using digital platforms for data entry and reporting.
  • Ability to maintain confidentiality and adhere to data privacy laws.
  • Understanding and commitment to cultural competency and providing services to diverse populations.
  • Completion of required training, including child abuse and neglect reporting certification.
  • Previous experience in a health, social services, or community outreach setting preferred.
  • Familiarity with screening tools, health data collection, or case management preferred.
  • Experience working with refugee and immigrant populations, as well as non-English speakers preferred.
  • Multilingual candidates are strongly encouraged to apply. Priority languages in addition to English are Spanish, Arabic, and Burmese, but other languages are helpful.
  • Candidate must be eligible to work within Medicaid and Medicare programs (we will conduct exclusion checks monthly).

  • Work time may be split between office settings or outside meetings. Local travel is required. Light physical effort in an office environment.
  • Frequent computer use.
  • Primarily sedentary work with occasional bending and reaching.

  • Maintain a high energy level; comfortable performing multi-faceted projects in conjunction with day-to-day activities.
  • Regularly required to speak clearly and hear the spoken word as well.
  • Regularly required to read and write clearly.
