Salary: $21.84
Under limited supervision will perform a variety of HHSC IDD Local Authority Performance Contract functions. Development and maintenance of public/private provider relationships consumer enrollments into HCS, TxHmL, and State Supported Living Centers. Responsible for transition planning activities for people leaving Nursing Facilities and State facilities, which includes interaction with NF, SSLC, and State Hospital staff. Will serve as the LIDDA's Enhanced Community Coordinator. Will perform PASRR evaluations and periodic PASRR Habilitation Coordination activities. Work involves independent judgment, good interpersonal skills, computer skills, and travel within the service area and state.
Position Requirements:
Required: Bachelor's degree in a social field, or an associate's degree in a human service field, or a high school diploma/GED with 1 year experience working with people with IDD.
Must have and maintain a Valid Driver's License and driving record that is acceptable for insurability through our insurance carrier.
Must pass background check and drug screening.
Applicants must reside within 70-mile radius of duty site
