Job Title: Patient Outreach Coordinator
Job Summary: The Patient Outreach Coordinator is responsible for actively engaging potential patients through various outreach methods, educating them about available healthcare services, addressing concerns, and facilitating their access to care. This role involves scheduling appointments, providing necessary information, and connecting patients with appropriate healthcare providers within our facility.
Key Responsibilities:
- Maintain accurate records of outreach activities, including patient contact information, outreach methods, and outcomes.
- Follow up with patients to ensure they are actively engaged in their treatment plans and address any barriers to care.
- Contact potential patients through phone calls and respond to patient inquiries regarding treatment and appointment scheduling.
- Update provider rosters, EPRG, and Privia Engage systems.
- Ensure the accuracy of primary care provider information for WellMed and Humana patients.
Transition of Care Responsibilities:
- Schedule timely follow-up appointments with primary care providers or specialists to monitor patient progress and address any concerns.
- Ensure proper medication management and coordinate a patient’s healthcare needs as they transition from hospital to home.
Additional Responsibilities:
- Track down retina appointments and colonoscopy reports for quality measure purposes.
- Work with spreadsheets from the Privia Population Health team.
- Assist new hires with training and authorization processes.
- Float between check-in, front office, or back office duties as needed.
Required Skills and Qualifications:
- Excellent communication and interpersonal skills to build rapport with diverse populations.
- Strong organizational and time management abilities to manage outreach campaigns effectively.
- Proficiency in customer relationship management systems, particularly Privia Engage, to track patient interactions and outreach efforts.
- Knowledge of healthcare practices, insurance plans, and medical terminology.
- Ability to work independently and collaboratively as part of a healthcare team.
- High school diploma or GED required.
- NRCMA or CMA certification required for transition of care responsibilities.

Pay: $15.00 - $17.00 per hour
