This position resides in the Team-Based Care department at West Wichita Family Physicians (WWFP). The nurse practitioner will be responsible for ensuring WWFP patients are receiving appropriate preventive care through completing wellness visits with our aging patient population. These visits are completed in-office, virtually and at the patient’s home. Wellness visits are intended to provide patients with an annual comprehensive snapshot of their health status and a plan moving forward to keep them as healthy as possible, for as long as possible. The annual visits provide an opportunity to address preventive health issues, appropriate testing and cognitive and environmental health assessments. All of the information gathered during the visit is organized and communicated to the patient’s primary care physician. This position will coordinate necessary follow-up care for patients as needs are identified during visits and is an important member of the primary care team.
This position will be responsible for meeting patient care and quality goals associated with annual wellness visits.
