What are the responsibilities and job description for the Hospital Liaison position at Westbrook Health Services, Inc?
Nature of Work:
The Hospital Liaison performs a variety of duties associated with the admission and discharge of client's from inpatient facilities. This role is responsible for ensuring that client's who wish to follow up with Westbrook Health Services are given a warm hand off before they leave the inpatient facility. This position is responsible for establishing and maintaining a great image and excellent working relationships with both outside agencies and all WHS departments and locations so that there are no barriers when it comes to coordinating care for our clients.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.
- Responsible for scheduling all aftercare appointments, requesting and reviewing medical records, and uploading all needed documentation into MyEvolv.
- Making appropriate internal referrals.
- Participating in staffings with state psychiatric hospital via phone or Google Meet.
- Facilitating weekly High Risk meeting via Teams.
- Communicating with inpatient and outpatient treatment teams to coordinate discharge planning.
- Sending out and keeping Inpatient Spreadsheet, High Risk Reports, Involuntary Discharge Reports, Monthly Staffing Reports, and Conditional Release Plans up to date.
- Following up with clients who have missed appointments.
- Establishes/maintains current files for reporting, verify for accuracy when process is completed.
- Complies with federal, state and company policies, procedures and regulations.
- Maintains confidentiality of all information and complies with HIPAA regulations.
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.

Education and/or Experience:
- High school diploma/GED required
- Bachelor’s Degree preferred, or experience working with community agencies.
- Must pass Criminal Investigation Background and DHHR background checks.
