What are the responsibilities and job description for the Youth Suicide Intervention Specialist position at Westbrook Health Services, Inc?
Summary: Provides prevention and intervention type community based services to improve care and outcomes for youth at risk for suicide and their families.
Essential Duties and Responsibilities:
· Work collaboratively with other agencies who provide services to adults.
· Attend local and state meetings related to adult services and suicide prevention.
· Provide outreach to adults and their families related to suicide prevention, intervention, treatment and recovery.
· Provide supportive counseling services within scope of training and competencies.
· Provide case management, including linkage/referral to needed services.
· Provide transportation to appointments and supports, as needed by the consumer.
· Provide crisis assistance, as needed by the consumer.
· Assists with Continuous Quality Assurance and participates in Utilization Management.
· Maintains confidentiality of all information and complies with HIPAA regulations.
· Comply with federal, state and company policies, procedures and regulations.
· Demonstrate knowledge and support of Westbrook Health Services’ mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.
This position requires travel between the eight counties served by Westbrook.
