General Statement of Duties:
Full time position (35 hrs/wk) aimed at risk mitigation and prevention of institutionalization for seniors and younger disabled individuals through care management services. Daily tasks include conducting comprehensive in-home assessments of seniors or younger disabled individuals to identify strengths and areas of need. Care Plan and goal development and implementation of services will help support an individual to remain safely in their homes. A care manager will provide ongoing monitoring, advocacy and support within an atmosphere of person centered planning. This position acts as the primary service contact with the senior or younger person with a disability (or families). Care Managers may be required to help the client/family with required paperwork to secure and maintain benefits including but not limited to Medicaid, Medicare, housing, and transportation. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.
Qualifications:
Candidates must be willing to work on site from the office during the initial 6 month period in between field work. Remote work is available to eligible employees after successful completion of this period.
Applicant must possess Bachelor’s or Master’s degree in Social Work or a related field; LMSW or LCSW preferred, or a Bachelor’s degree in Nursing with current active CT license. All applicants must have two years of demonstrated related experience working in health and human services. Acquired Brain Injury Care Managers must possess an LMSW, LCSW, or a Master’s degree in Human Services, Counseling or Rehabilitation Counseling, in addition to two years demonstrated experience. Vehivle and valid CT driver’s license required as this position requires travel. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. Assessing individuals in their homes, a nursing or other skilled facilities will be required as needed. Second language is a plus.
