Summary :
At Western Connecticut Home Care, we are seeking individuals to become part of our exceptional home healthcare team. Responsibilities :
- Works under the direction and supervision of the licensed Physical Therapist in the home environment practice setting.
- Performs Physical Therapy procedures ad related tasks under the direction of the supervising Physical Therapist.
- Provides patient / family education and training.
- Performs routine operational functions, including superision of Home Health Aide.
- Documents visits and completes documentation within 24 hours of visit and within agency guidelines.
Work Type : Full-Time
Work Shift : Monday-Friday; 8 : 30AM-5 : 00PM
