- Provide hospice care and support for patients with terminal illnesses.
- Conduct triage assessments to prioritize patient needs effectively.
- Offer family planning education and resources to patients as needed.
- Deliver home care services, ensuring a safe and supportive environment for patients.
- Maintain accurate patient records and documentation in compliance with healthcare standards.
- Collaborate with healthcare teams to develop and implement individualized care plans.
Skills
