Overview: Western Home Communities is seeking a passionate and dedicated Clinical Services Coordinator (Registered Nurse) to join our Assisted Living team at our Cedar Falls community. We are looking for a detail-oriented, compassionate individual with a strong desire to create fulfilling lifestyles for our residents. In this role, you will have the opportunity to provide high-quality nursing care while making a meaningful impact on the lives of those you serve. This is a PRN opportunity for someone who is looking for experience and flexibility by working the hours you choose. Western Home Communities is looking for someone to fill in periodically to primarily complete resident assessments.
Key Responsibilities:
Resident Care Management: Guide and direct resident care as part of the management team, ensuring all residents under your care receive the appropriate services and attention.
Individualized Care Plans: Create and maintain personalized service plans for each resident based on assessments and reviews of their needs.
Effective Communication: Coordinate with physicians and the immediate supervisor to communicate resident needs and treatment plans.
Admission Coordination: Assist in the preparation and planning of resident admissions, collaborating with family members and others to ensure comprehensive nursing care.
Emotional and Physical Support: Provide comfort and safety to residents, offering both emotional and physical support, and extending support to families.
Team Leadership: Supervise, consult with, and teach Resident Assistants, CNAs, and Medication Managers to ensure effective team performance.
Qualifications:
Graduate of a state board-approved RN school
Currently licensed as an RN in Iowa
CPR certification (or willingness to obtain)
Strong attendance and reliability
Ability to work well in a team and foster collaborative relationships
