Position Overview :
As an Advanced Practice Provider for our agency, youll have the autonomy to provide full-spectrum primary care in a low-stress, high-impact environment. Youll work closely with patients to manage their health, promote wellness, and treat acute and chronic conditions without the high patient volumes or burdensome bureaucracy. We offer our patients house calls just like medicine used to be.
Key Responsibilities :
Provide primary care services to a dedicated panel of patients
Conduct physical exams, order and interpret diagnostic tests, and develop treatment plans
Establish and maintain long-term patient relationships
Offer health promotion, disease prevention, and education
Work collaboratively with other providers in a supportive, team-oriented environment
Practice evidence-based medicine with a focus on holistic and preventative care
Work collaboratively with the marketing team to grow our practice and serve more patients
Qualifications :
Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure
Experience in primary care or family medicine preferred
Passion for partnering with your patients and patient-centered care
Ability to build trust and strong relationships with patients
Excellent communication and clinical skills
Independent, self-motivated, and proactive approach to care
Ability to self-schedule and to prioritize patient needs
Home health or hospice experience is encouraged
