What are the responsibilities and job description for the Youth Targeted Case Management position at Western Montana Mental Health Center?
A Targeted Case Manager (TCM) assesses needs, develops a service plan, coordinates services and monitors progress for the family system with an emphasis on youth with serious emotional disturbances (SED). The TCM assists, advocates, and empowers the family to help the youth obtain the necessary services to remain in school, in the home, and in their community while focusing on their social, emotional, behavioral and basic needs.

Minimum Qualifications:
- Graduation from an accredited college with a degree in Psychology, Social Work, Education, or a related field.
- Minimum of two (2) years’ experience preferred.
- Ability to pass a background check upon offer of employment
- Valid current driver’s license with good driving record.
