What are the responsibilities and job description for the Program Manager - Social Care Program position at Western New York Integrated Care Collaborative?
Overview:
We are seeking a dynamic and experienced Program Manager to oversee social care programs being delivered by our community-based partners (i.e. care coordination, meal delivery, social services) . The ideal candidate will have a strong background in care coordination, network management, change management, and training.
RESPONSIBILITIES INCLUDE:
- Oversight and coordination of multiple related programs
- Includes: providing trainings, chart reviews, program coordination, reporting, quality assurance, credentialing, and other tasks, as needed
- Provide Technical Support to contracted Delivery partners.
- Provide Program Services, including a case load of clients, as needed
- Develop new programs and processes with Management Team
- Provides support for assigned Committees and / Workgroups of the Network.
- Professionally represents WNYICC on network-wide initiatives.
- This position will include both field-based and office-based responsibilities. Work From Home / Remote office is an option for office-based responsibilities, although in-person team meetings may be required. Travel may include local, regional, and state-wide for in-person responsibilities and out-of-state for conferences.
QUALIFICATIONS:
- Requires a minimum of a bachelor’s degree in public health, social work, psychology, or related fields (master’s preferred); or 5 years equivalent work experience with references.
- Must have strong computer/technology skills including: electronic health record, Microsoft Office-Suite, Outlook, and more.
- Ability to lead group presentations and conduct trainings.
- Ability to relate and communicate effectively to diverse groups of people from all social and economic segments of the community with a track record of building authentic, constructive relationships with others.
- Experience providing care management, case management and / or care coordination.
- Experience providing trainings and quality control management.
- Experience with programs and services addressing health equity, especially housing and transportation.
- Team player who enjoys working both collaboratively and independently.
- Must have a reliable form of transportation, as a significant portion of the programs will be conducted in the field throughout the Western and Central New York regions.
- Demonstrates excellent customer service on phone, in-person, via email and in all aspects of employment at WNYICC.
- Communicates in a positive, helpful manner both internally and externally to partners.
- Assists Team in a wide array of responsibilities from event planning to reports.

- SALARY: $62,500 - $65,000 plus benefits
