Title: CPT Case Manager

Overview:
The CPT Case Manager is a full-time, exempt position responsible for coordinating and managing activities within the Career Pathways Training Program. This program is a component of the New York Health Equity Reform (NYHER) 1115 Waiver. The CPT Program is an education and training program designed to promote health equity and to create a reliable workforce pipeline to address workforce shortages by funding education and training for health, behavioral health, and social care workers throughout NYS. All program participants will receive financial assistance (including tuition coverage), educational case management, training support, and job readiness and placement assistance. This program aligns with the Western New York Rural Area Health Education Center’s (WNY R-AHEC) mission and goals. The role requires strong communication skills, compassion, mentorship skills, and the ability to work independently while contributing to team projects. Description: In alignment with the organizational mission, vision, goals, values, and strategies, the case manager is responsible for assisting in the execution and maintenance of one or more organization projects.
Reporting Relationship/Accountability:
The CPT Case Manager reports to the Program Manager and Associate Director.
Responsibilities:
- Develop individualized case management plans for clients, outlining specific goals, actions, and timelines.
- Coordinate and manage casework for individuals, ensuring timely follow-ups and progress tracking in alignment with program objectives.
- Serve as a primary point of contact for clients, helping them access services and resolve challenges.
- Be aware of social service agencies and community partners that may be able to assist clients.
- Develop outreach materials and presentations to promote case management services to the community and other stakeholders.
- Regularly track, enter, and maintain client information and case updates in the AHEC Tracker database.
- Contribute to the development of project objectives and outcomes.
- Contribute to R-AHEC’s social media and website content by providing updates on program activities and successes.
- Attend and represent R-AHEC at community meetings, conferences, and workgroups, as needed.
- Ensure compliance with all program standards, policies, and reporting requirements.
- Provide training or guidance to interns, staff, or community partners on case management practices and resources available to clients.
- Regularly assess program performance and adjust services as needed to achieve program goals.
- Perform additional tasks and duties as required by the Program Manager, Associate Director, and/or Executive Director.

Education & Experience:
- Bachelor’s degree or equivalent combination of education and experience.
- Experience in case management, social work, public health, or community-based services strongly preferred.
- Previous work in healthcare or social service settings highly preferred.
- Experience working with diverse populations and understanding of health disparities.
Skills & Proficiencies:
- Excellent written, verbal, and interpersonal communication skills, including the ability to communicate effectively with clients, healthcare professionals, and community organizations. High comfort level with public speaking.
- Ability to present materials in an engaging and interesting manner.
- Strong organizational and time management skills, with the ability to manage multiple projects and tasks simultaneously.
- Ability to work independently and as part of a team, adapting to changing situations and needs.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and database management systems.
- Critical thinking skills and the ability to exercise sound judgment when addressing client needs.
- Strong analytical and problem-solving skills.
- A good understanding of educational processes.
- A passion for public health, social services, and community improvement.
- Ability to navigate healthcare and social service systems and understand relevant policies and regulations.
