This contract position includes:

The Care Coordinator will provide home based Wraparound Services as a team with a Family Peer Recovery Support Specialist within Texas, Cimarron, and Beaver counties. If the candidate is not already certified as a Behavioral Health Case Manager II, the agency will pay for the certification. Minimum qualifications include:
1) HS Diploma or GED AND 36 months of Behavioral Health (BH) experience
OR
2) 60 college credit hours in any field AND 12 mo. of BH experience.
OR
3) Bachelor's or master's in any field AND 6 mo. BH experience.
OR
4) Bachelor's or master's degree in a BH related field.
OR
5) RN with BH experience.
OR
6)LBHP Licensure candidate
OR
7) CADC
OR
8) certified as a BH Case Manager II
