Job Overview
Western Prairie Human Services, an Equal Opportunity Employer, seeking a compassionate and dedicated Case Aide Community Support Worker to join our team. This position will be based in the Elbow Lake office.
The Case Aide Community Support Worker will play a crucial role in providing support to individuals in various settings and home care environments. This position requires a strong commitment to adults who are eligible for Community Support Program and the ability to work effectively with individuals. The ideal candidate will be proactive, empathetic, and possess excellent communication skills.
A bachelor’s degree with coursework in social work, psychology, sociology, or related field is preferred but not required. A minimal of 1 year experience preferred in previous related social work, or equivalent combination of education and experience.
Responsibilities
- Performs Community Support Program work in the communities to help individuals maximize independence
- Provides transportation to clients for the purpose of the Community Support Program
- Prepares and assists with paperwork, reports, and processes
- Coordinates community outreach and meetings
- Completes program referral coordination and data requests
- Scans documents into the electronic filing system
- Prepares files and folders
- Other Duties as assigned.
Skills
- General knowledge of the principles and practices of public social service organizations.
- General knowledge of agency programs and operations and other community resources.
- Ability to analyze data and exercise sound judgment in arriving at conclusions.
- Ability to follow complex oral and written instructions; ability to manage workload and meet deadlines.
- Ability to communicate effectively orally and in writing; ability to prepare clear and concise reports.
- Ability to establish and maintain effective working relationships with clients, associates and the general public.

This is a full-time position, non-exempt, union position. Anticipated beginning pay is $24.33 per hour. Health insurance, pension and an array of benefits are offered.
