Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. Reports to: Supervisor, Aging and Disability Resource Center
FLSA Status: Exempt
Position Overview: The role of Community Resource Consultant is to initiate the process of evaluating the individual/consumer needs and providing information regarding available options.
Job Duties:
- Complete face to face assessments in the community with Consumer (and caregiver/contact) to determine individual’s level of care.
- Communicate and collaborate with caregivers, healthcare and service providers, and any other formal and informal systems to accurately determine and meet the individual’s needs.
- Provide options and referrals to meet individual’s needs if not pursuing Medicaid Waiver
- Document in appropriate software system according to policy.
- Work cooperatively and constructively with internal WRAAA staff, and with formal and informal systems
- Advocate on client’s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization.
- Ability to complete assessments during core business hours and occasionally early evening or weekend hours as needed to meet the needs of the community.
Requirements:
- Current licensure as a social worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing.
- Two years of experience in home care, geriatrics, home health care, medical social work, or other substitute as approved by the Ohio Department of Aging.
- Basic knowledge of major chronic illnesses, disease processes, normal aging, and basic knowledge of medical terminology.
- Excellent verbal and written interviewing and assessment skills.
- Knowledge of local resources for older adults.
- Ability to communicate in a collaborative, effective manner with others and maintain good working relationships.
- Intermediate computer skills including Microsoft Office applications.
- Ability to complete job duties by telecommunication from home and field.
- Ability to record information in an accurate, timely manner and meet each program’s time frames.
- Ability to use and will be trained in the Ohio Department of Aging and Ohio Department of Medicaid documentation systems.
- Ability to use and transport a laptop computer and case, therefore must be able to lift/carry a minimum of twenty (20) pounds.
- Ability to ambulate enough to access private homes/locations which may not be readily accessible.
- Must have a car available during working hours, as well as the ability to legally drive a car.
