It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network works together to create opportunities for Ohioan's to receive needed home and community services and supports; and to age successfully in their own homes and communities.

Position Summary: The Long-term Care Consultant meets with consumers/caregivers seeking information about options available to meet long-term care needs or consumers that are seeking Medicaid payment for admission or continued stay in a nursing facility. The LTC Consultant will assess and evaluate long-term care needs, provide options and assistance with locating and connecting to long-term services and supports that promote safety and independence in the community. The LTC Consultant will complete Community Living Specialist assessments for current NF residents to explore desire and ability to integrate back to community.

Primary Job Duties:
- Obtain and maintain Long Term Care Consultant Certification from the Ohio Department of Aging.
- Meet with the consumer and/or caregiver to determine long-term care needs in accordance with the Ohio Administrative Code. Meetings can occur in a Nursing Facility, consumer/caregiver home or via telephone.
- Focus on individual’s needs, circumstances and values and provide options to meet the consumer’s long-term care needs, including payment sources, factors to consider when choosing among available programs, services and benefits, and opportunities for maximizing independence and self-reliance.
- Develop an individualized community care plan between the consumer/caregiver regarding identified needs, goals and interventions.
- Make appropriate referrals to community providers, waiver programs, VA, Home Choice, etc. with agreement from consumer/caregiver.
- Assist with completing applications on consumer’s behalf including but not limited to MIPPA, Medicaid, SNAP, Veterans Benefits, Home Choice, etc.
- Provide consumer/caregiver with a written summary of identified needs, options to meet those needs and referrals made on behalf of the consumer.
- Advocate on consumer’s behalf.
- Report abuse, neglect or exploitation of older adults as required by Ohio law when appropriate.
- Maintain confidentiality according to Agency and HIPPA policies.
- Document in appropriate IT platform according to policy and quality compliance standards.
- Adhere to daily and monthly productivity standards.
- Work cooperatively and constructively with internal WRAAA staff and formal/informal systems.
- Promote positive employee relations and fosters excellence in customer service.
- Participate in and contribute toward accomplishment of Agency and department goals.
- Abide by professional code of ethics established by licensure, if applicable.

- Minimum requirement includes Bachelor’s Degree and one year of experience in home care, long term care, medical, behavioral health or geriatrics.
- Knowledge of major chronic illnesses, disease processes, normal aging and medical terminology.
- Familiar with Medicare, Medicaid and community resources in the five-county area of Geauga, Lake, Lorain, Medina and Cuyahoga Counties.
- Ability to become OSHIIP Certified within one year of hire.
- Ability to effectively communicate and build rapport in person, by phone, text or email.
- Ability to record information concurrently with interview in an accurate and timely manner to meet program requirements and timeframes.
- The ability to independently solve problems including the ability to analyze, evaluate and implement reasonable courses of action based on available information.
- Intermediate computer skills including Microsoft applications of Outlook, Teams, SharePoint, Excel and Word.
- Ability to transport a laptop computer, case and documents, therefore must be able to lift/carry minimum of 20 lbs.
- Ability to ambulate enough to access private homes and facilities which may not be readily accessible.
- Must have a valid driver’s license and access to a vehicle during working hours. May occasionally be asked to complete consultation outside of regular office hours if requested by consumer/caregiver.
