Position Overview:
Recovery Management includes coordinating all services received by an individual and assisting the individual in gaining access to needed Medicaid State Plan and 1915(i) services (Specialized Recovery Services-SRS), as well as medical, social, educational, and other resources, regardless of funding source. Recovery Managers are responsible for monitoring the provision of services included the Person-Centered Plan to ensure that the individual’s needs, preferences, health, and welfare are promoted.
Job Duties:
- The Recovery Manager will conduct initial assessments as assigned. Complete face to face assessments in the community with Consumer (and caregiver/contact) assessing the needs, service goals, objectives and personal preferences to enroll into the program. Provides supporting documentation to be considered by the independent entity in the review and approval process.
- The Recovery Manager will manage an individual case load.
- Annually assessing the individuals program eligibility and assisting the individual with retaining HCBS and Medicaid. Assists the individual with fair hearing requests when needed and upon request.
- Updates the assessment, as applicable, and the Person-Centered Plan, based on information discovered during ongoing monitoring. Revisions to the Person-Centered Plan should occur no less frequently than annually.
- Scheduling, coordinating, and facilitating meetings with the individual, which should include the individual’s transdisciplinary team, informal support, and other providers servicing the member on a regular basis such as community mental health teams.
- Authorizing services in the type, amount, scope, frequency and duration to meet the individual needs.
- Linking and referring the individual to service providers, educational resources, and support programs appropriate for individual and their informal support system, including non-HCBS Medicaid, Medicare, and/or private insurance or other community resources.
- Actively participates in the care planning process as a member of the trans-disciplinary team.
- Monitoring the delivery of all services identified in the individuals Person-Centered Service Plan. Through the ongoing monitoring process, if there is discovery of a significant change event (e.g., inpatient hospital admission), the Recovery Manager will follow the RM Guide. Transition planning for significant changes including transitioning from SRS to a waiver or nursing facility.
- Monitors health, welfare, wellness, and safety through regular monthly contacts (calls and visits with the individual, paid and unpaid support, and natural support) wherever the individual lives, works, or has activities.
- Identifying and reporting incidents, assist the individual to develop a prevention plan to reduce the risk of reoccurrence.
- Participates in any activities related to quality oversight and provides reporting as required.
- Assist the supervisor with completing required audits, case reviews, and adverse outcome responses.
- Respond to urgent/emergent needs for a recovery manager’s visit if the assigned recovery manager is not available.
- Maintain minimum proficiency in all requisite computer applications and software.
- Maintain consumer confidentiality according to agency policy.
- Report abuse, neglect, or exploitation of older adults as a mandated reporter as required by Ohio law.
- Abide by the professional code of ethics established by licensure.
- Participate in trainings, meetings, in-services and continuing education opportunities; also participates in Ohio Home Care Waiver-SRS trainings and meetings as requested by supervisor or manager.
- Meet time frames for timely and accurate completion of documentation, including use of the Guiding Care IT system.
- Ability and willingness to travel to community and consumer locations within our five county service areas (Cuyahoga, Lorain, Geauga, Lake and Medina counties).
- Perform other duties as assigned by supervisor as needed to meet agency goals.

Requirements:
- An RN or bachelor’s in social work, counseling, psychology, or a related social service field with a minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI) or have a minimum of one year post degree experience working with individuals with diagnoses chronic conditions.
- Training in administering ANSA, person-centered planning, evaluating HCBS living arrangements, incident reporting and meeting state conflict of interest standards.
- Applicant must demonstrate the knowledge and skills necessary to provide the level care management/assessment appropriate to the scope of program responsibilities.
- Experience with any of the following is desirable: mental health patients, or physically disabled patients.
- Applicant must possess a valid driver’s license and proof of automobile insurance.
- Applicant must pass a criminal background check and drug screen.
- Applicant must have basic computer skills, including Microsoft Office applications
