We are seeking Qualified Mental Health Professionals for our
______Waverly Nursing Home Pilot Program______________ department to work PRN (as needed)
**Gero-Psychiatric, Mental Health, or ID/DD experience is preferred for this position. **
Responsibilities and Duties for QMHP:
- Teach consumers appropriate strategies to manage behavioral and mental health symptoms
- Assist with consumer service plans related to identified needs and goals
- Assist consumers in gaining access to appropriate medical, psychiatric, social, educational, vocational and other support and services
- Provide consumer training/reinforcement, linkage to supports/services
- Advocate on behalf of consumer, family and providers
- Monitor/assess service delivery
- Behavior management and crisis intervention.
Qualifications and Skills:
- Bachelor’s Degree in a Human Service field
- One year experience working directly with individuals with intellectual disabilities, mental illness, substance abuse or who are dually diagnosed.
- QMHP-C or QMHP-A certified by the Board of Health Professions
- Valid Driver’s License with acceptable DMV record.
Work Schedule:
Must be able to work Days, Nights/Overnights, and Weekends
