We are seeking qualified Case Managers to work with individuals in need of case management but don’t have the funding.
** Substance Abuse and Mental Health experience is preferred. **
Location: Suffolk, VA
Responsibilities and Duties for Case Manager:
- Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP
- Assist and provide follow up instruction, education, and counseling to consumers/families who do not have insurance to cover Mental Health Case Management.
- Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
- Manage consumer records according to all Licensure and Medicaid standards.
Qualifications and Skills for Case Manager:
- BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field
- One year of clinical experience in a Human Services field
- QMHP or QIDP preferred
- KSA's (Required)
- Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter
