JOB TITLE: CASE MANAGER – DCMH RAP
DEPARTMENT: SUPPORTIVE SERVICES
FLSA: NON-EXEMPT
LOCATION: WESTCHESTER COUNTY
REPORTS TO: SERVICE COORDINATOR
SALARY: $40,000.00 - $45,000.00
SUMMARY:
Directly provides case management, counseling, life skills coaching, crisis intervention and programmatic tasks for formerly homeless families headed by an adult with a psychiatric disability. Covid-19 vaccination is required.
DUTIES & RESPONSIBILITIES:
- Responsible for individual caseload of single and family adult participants.
- Oversees home finding process including apartment finding and/or referrals for housing assistance. Responsible for processing move-ins/move-outs.
- Completes initial intakes and assesses housing readiness of potential clients; designs services plan for each participant on caseload, coordinates interventions and services for clients.
- Manages lease executions, internal accounting processes and entitlement requests from the Department of Social Services.
- Meets regularly with participants to update service plans, while assisting them in obtaining needed services/benefits.
- Works with program participants to pursue alternative housing.
- Schedules and performs home visits on a regular basis, monitoring housing related issues.
- Establishes and maintains positive relations with each landlord.
- Assists the Service Coordinator in managing rent payments and turnovers.
- Tracks services provided to clients and completes required program reports. Attends all relevant community/service provider meetings.
- Communicates with all collateral agencies to ensure participant compliance with service plan.
- Documents case management activities in accordance with agency and funder guidelines and procedures.
- Collects service data and participates in program evaluation and quality assurance activities.
- Provides crisis intervention and stabilization as needed. May require some after hour’s follow-up to emergency situations.
- Contributes to monthly, quarterly and annual reports.
- Other duties as required.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
BSW or 5 years’ relevant experience required. Previous experience in human services; working with substance abuse, mental health and homeless populations. Housing relocation experience helpful. Must have excellent written and verbal communication skills. Must have car and valid drivers’ license. Word and Excel proficiency a must. Requires travel throughout Westchester County. May require some evening and weekend hours
