This includes butis not limited to promoting and enhancing relationships between residents,their families and the support systems and resources inside or outside thecommunity. - Abasic understanding of how to support those living with mental health,alcohol / drug addiction, post-traumatic injuries, physical disabilities, anddementia Defining Success : As a member of our team, your primaryresponsibilities are : Advocacy and Support : o Developone-to-one relationships with residents and families as needed for support o Whennecessary, refer residents to social, health, mental health, and communityagencies and ensure documentation is in the resident's chart regarding theresults of referrals o Upondischarge, coordinate family and community resources as needed to solvefinancial needs and promote emotional security o Possessan understanding of resident rights and serve as an educator to residents,families, and employees concerning resident rights o Utilizethe PETI Program and other community resources to ensure eligible residentsreceive glasses, dentures and other services o Participatein the theft / loss program as well as formal and informal grievance procedures o Completedischarge planning and transfers in accordance with discharge policy o CompleteSocial Services portion of the resident's MDS within required timeline o Completea social history of a resident within state required timeline o Documentrelevant interactions with residents and / or families in resident chart asrequired by state standards o Involvementin pharma and non-pharma interventions and behavior tracking o Makeplanned one-to-one visits with residents and document progress in chart o Assistin the development and implementation of relevant policies and procedures o Followthrough with PaSSRR recommendations Empowerment : o Assistresidents to find meaningful work and a sense of purpose in the community o Developthe social care component of the Care Plan for each resident. o Ensureresident and staff participation in their care plan o Collaboratewith Residents and staff to develop non-pharmalogical interventionsMediation : o Supportresidents and families in dealing with feelings about death or dying and otheremotional, mental, environmental or physical limitations; provide resourcesinside and outside of the community; working knowledge of advanced directivesand palliative care o Collaboratewith nursing to ensure the proper care of residents o Actas a liaison with social, health and community agencies o Investigateall reports of potential abuse and neglect Safety : o Befamiliar with disaster planning policies and procedure HIPAA : o Comply with HIPAA and Confidentiality policy atall times ? Occurrences : o Promptly reports any occurrences or potentialabuse immediately General o Approachall encounters with residents, guests, and employees in a friendly,service-oriented manner o Maintainregular attendance as required o Maintainhigh standards of personal appearance and grooming, which include wearing theproper uniform and nametag when working o Complyat all times with the standards, policies, and regulations to encourageproductive, safe, and efficient community operations o Attendin-service education, care conferences, and meetings as required o Otherduties as assigned Pay Range : $20.00-$25.00 / hr.
