Salary : SUMMARY :
The Outreach and Enrollment Coordinator serves as the point of contact for individuals and / or families regarding health insurance enrollment via the Federally Facilitated Marketplace as created under the Affordable Care Act (ACA). This person is responsible for addressing the health and social service needs of uninsured and / or low-income individuals and families within the community by conducting outreach to inform and assist them with enrollment in the Federally Facilitated Marketplace, providing referrals to health and other supportive services, and coordinating outreach enrollment activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
1. Visit with individuals and their families to learn details about their health and provide information on available resources as it pertains to Healthcare.gov eligibility.
2. Document and maintain records of encounters with individuals and / or families.
3. Provide comprehensive and culturally sensitive information about public health insurance programs.
4. Provide an application packet and a brief overview the Sliding Fee Scale program
5. Provide information about our free pharmaceutical program and give patients an opportunity to apply for this program.
6. Assist eligible individuals and families with completing their application and answer questions about the necessary documentation requested.
7. Ensure the confidentiality of all applications, records, and information received.
8. Conduct follow-up with individuals and families on the progress of their enrollment
9. Coordinate outreach and enrollment efforts by working collaboratively with community partner organizations, local churches, labor groups and other community entities.
10. Participate in outreach and community events.
11. Track eligibility, approval and utilization. Maintain a tracking Log and provide reports of activities to management, upon request.
12. Participate in NCCHCA workgroup meetings.
13. Maintain certification for Certified Application Counselor (CAC).
14. Manage Dental voucher Program
15. Manage transitional outreach programs.
16. Plan all major events for the company.
17. Maintain food closet for patients with food insecurity.
18. Ensure that appearances and personal conduct are professional at all times.
19. Comply with Corporate policies regarding ethical business practices.
20. Maintain confidentiality and privacy at all times.
21. Represent WCHC in a positive and professional manner.
22. Maintain excellent attendance record.
23. Develop and maintain good working relationships within the organization and community.
24. Support principles of Patient Centered Medical Home (PCMH) by completing PCMH education modules / training which may include Patient Self Management, Care Coordination, Population Management, Whole Person Care, and Communication Skills.
Other Functions
Perform other related duties as required or assigned by the CFO incidental to the work described herein. Perform non-related duties, as needed, necessary for the smooth operation of the clinic at the direction of Administration.
QUALIFICATION REQUIREMENTS :
Basic computer skills including web-based applications and email.
Basic office equipment skills including fax, copier and scanner.
Strong interpersonal and communication skills.
Must be professional in appearance.
Must be able to exercise discretion in all matters related to the enrollment process.
Must be able to demonstrate understanding, compassion and respect for individuals and families who face a myriad of socio-economic challenges.
Must be able to handle multiple tasks.
Excellent work history.
Ability to pass periodic drug testing.
No convictions of serious crimes or felonies.
Willing to travel to outreach and enrollment sites.
Must have reliable transportation, valid Drivers License, and proper car insurance.
Knowledge of current HIPAA Rules and Regulations.
Team player.
EDUCATION / EXPERIENCE :
Two years prior experience working in community service, health care, or social service sectors with uninsured and / or low-income individuals and families. Preferred.
Knowledge about available program and community resources. Preferred.
Certified Application Counselor Certification, or willing to obtain.
LANGUAGE SKILLS :
Must be able to read, write and speak English. Basic Spanish preferred.
Ability to read, proof-read and interpret documents, instructions, and procedure manuals.
Ability to communicate effectively in person and via telephone.
MATHEMATICAL SKILLS :
Ability to apply basic mathematical principles when calculating figures and amounts.
REASONING ABILITY :
Ability to manage and solve patient problems with courtesy and sensitivity.
PHYSICAL / MENTAL DEMANDS :
While performing the duties of this job, the employee is regularly required to talk, hear, sit, stand, walk, climb, balance on two feet, stoop, kneel, or crouch. The employee is required to reach with hands and arms, and use hands and fingers to handle / feel objects, tools, or controls. The employee must be able to lift and / or remove up to 25 pounds. Vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Mental demands include the ability to learn, adapt to changes, become proficient quickly, pay close attention to details, exhibit a courteous and professional demeanor, ability to deal with stress of meeting deadlines and adhere to company policy and procedures.
