Job Requirements :
A valid Registered Nurse license and a Bachelor's degree in Nursing, two (2) years of Nurse manager or ADON experience, long term care experience and; or combination of education and experience. Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form, possess high-quality verbal and written communication ability, strong customer service and interpersonal skills. Must be proficient in the use of various computer applications and be knowledgeable on all state and federal health regulations. Current CPR certification required. Must maintain current Infection Preventionist certification as required.
Job Summary :
This position is primarily responsible for assisting the Director of Nursing in the overall operations of the Nursing Department in accordance with current state and federal regulations. Must be competent in understanding, and utilizing the QAPI process. Must be able to accurately complete nursing assessments, comprise nursing interventions and assist staff as needed on the nursing center floor. Assistant Director of Nursing must be available to fill in when the Director of Nursing is unavailable.
Key Responsibilities :
Nursing Operations
Assists in supervising the delivery of all nursing operations and functions of Health Center on a daily basis and ensures effective and efficient resident care.
Delegates responsibilities, duties, or activities to designate nursing staff when appropriate. Follows up on all delegated responsibilities to ensure completion.
Ensure staff carry out resident care policies on a daily basis.
Creates written or oral recommendations to the DON concerning the operations of the Nursing department for areas of improvement.
Conducts daily rounds of all nursing units to ensure the nursing staff is providing the highest level of care to residents.
Assists in reviewing and identifying all new resident medical needs. Makes recommendations on resident needs and documents in resident chart or file.
Assures that the Nursing department is maintained in a clean and safe manner for the highest level of resident comfort and satisfaction.
Participates n quality assurance performance improvement (QAPI) committee meetings to identify issues and to develop and implement appropriate plans of action to correct identified problems.
Participates in infection control programs.
Assist with interviewing recruitment and on-boarding of new nursing staff.
Care Plan Development
Works with the Care Plan team to develop individualized resident directed care plans for all residents of Ingleside.
Ensures that the resident directed care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
Evaluates each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
Coordinates with other Care Plan team members and schedules the resident assessment / care plan meetings.
Contacts and assists in scheduling participation by outside members of the care plan team, including the resident's representative and / or other interested family members.
Ensures that care provided is in accordance with the residents wishes.
Participates in discharge plans as necessary
Works with the Care Plan team, as the leader, in developing a comprehensive resident assessment and resident directed care plan for each resident.
Attends various other committees of the community as required.
Coordinates, reviews, and revises the residents care plan by Care Plan team after each quarterly review or other assessment. Ensures that the care plan is evaluated and revised each time an assessment is done and when there is a change in the resident's status.
Updates the physicians and nursing staff of new or changed needs of residents. Documents all changes made in the updated care plan.
Supervisory Responsibility
Acts as a leader and manager of assigned positions; ensuring productivity, efficiency and accuracy in work performed.
Participates in on-call rotation during off hours, weekends, and holidays in coordination with DON.
Ensures initial and on-going training is provided to employees as needed; ensures cross-training is completed as appropriate.
Identifies unacceptable performance and behavior exhibited by employees and takes appropriate action; utilizes progressive discipline policy when appropriate and maintains documentation as requires.
Assures that all nursing staff follows established policies and procedures at all times.
Updates Resident Directed Care Plan procedures on an as needed basis
Assists and supervises resident intake during meal time.
