RN - LPN Charge Nurse (7P-7A)

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA is seeking a Registered Nurse (RN)  or Licensed Practical Nurse (LPN) to join our team on our Healthcare Team. Requirements

• Collect data and collaborate in the assessment of the Residents’ health status. Responsible for assessing residents post-fall and any skin issues.
• Develop relationships with Care Partners to seek as well as give clinical information about Residents.
• Collaborate in the development and modification of a Registered Nurse’s comprehensive nursing plan of care.
• Review resident clinical record; ensure accuracy of daily orders and documents scanned into the medical record. 
• Attend interdisciplinary meetings and work collaboratively with team members for POC.
• Implement the plan of care and delegated medical concerns.
• Answer questions regarding Resident’s welfare, care and general condition.
• Serve as an advocate for the Resident by communicating and collaborating with other health service personnel.
• Participate in the evaluation of a Resident’s response to an intervention.
• Communicate and collaborate with other healthcare professionals as needed. 
• Provide input into the development of policies and procedures to support Residents’ safety.
• Maintain records and charts and assure inventory of medications and narcotics.
• Maintain cleanliness of medication area in household: resident medication cabinets and refrigerator.
• Coach and collaborate with Care Partners in providing optimum healthcare for the Residents by reporting symptoms of the Resident’s condition(s) to the appropriate healthcare professional.
• Administer medications via orally, subdermal, and intramuscular and carry out treatment orders.
• Transcription of orders per policy.
• Ensure medications are transcribed correctly, and input all MD orders into medical record to ensure pharmacy receives orders in a timely manner.
• Properly and safely, store all medications received from pharmacy per policy.
