Westminster-Canterbury of the Blue Ridge (WCBR) is seeking a team member to join our Resident Services department supporting a lively 57-acre campus, 450 residents, and 300 associates.
The Independent Living Social Worker at Westminster-Canterbury of the Blue Ridge (WCBR) provides support and resources needed to enhance the well-being of the residents, families, and those involved in their lives.
\n- Assist and support the psychosocial needs of Independent Living residents, primarily during critical transitions involving losses associated with cognitive and/or physical decline while residing in Independent Living,
- Assess and respond to psychosocial needs of Independent Living residents and their families; collaborate with the social worker in HC and the social work coordinator in CL during transitions as well as dining, activities, etc. to meet identified needs
- Provide short-term individual and family counseling, and group work as appropriate
- Serve on committees and support groups; arrange for educational opportunities to address resident concerns; Attend IDT meetings and care plan meetings for residents temporarily in Healthcare, Assisted Living, and Vista
- Provide community referrals to community resources as needed
- The ability to establish rapport quickly and communicate effectively with residents and their families
- The ability to function effectively as a member of the Interdisciplinary Team
- The ability to conduct brief, evidence-based screening and to conduct or arrange for more detailed assessments when indicated
- The ability to create and implement integrated care plans
- Possess strong interpersonal skills including relationship building with associates, residents, families, and the greater Charlottesville community
- Current LCSW in the state of Virginia with Master’s Degree in Social Work
- Relevant experience with the aging population and their families
- Experience in a healthcare setting
