Job Purpose
The Home Health Director plays a pivotal role in ensuring the highest quality of care for our home health patients. With a focus on leadership, you’ll oversee the delivery of care, guide a dedicated team of clinicians, and ensure that patients and caregivers receive tailored, compassionate care. This is a hands-on, dynamic position where you’ll be instrumental in the development, implementation, and ongoing evaluation of care plans. Duties and Responsibilities
• Lead and inspire staff in providing exceptional patient care, including admissions, discharges, transfers, and service provisions.
• Collaborate closely with the Administrator on employee evaluations, coaching, and handling disciplinary matters.
• Oversee the implementation of individualized care plans by managing referrals, assigning cases, and ensuring integration of the necessary resources to meet patient goals.
• Audit and maintain accurate and up-to-date client records in compliance with agency policies and applicable guidelines. Regularly assess the effectiveness of care plans and participate in performance improvement initiatives.
• Conduct ongoing clinical assessments, developing and implementing comprehensive care plans tailored to patient needs, while adhering to professional standards and agency protocols.
• Collaborate with interdisciplinary teams to ensure holistic care for patients, including processing referrals and facilitating case conferences.
• Lead team meetings, in-services, and nursing meetings to maintain open communication and foster a learning environment.
• Supervise and mentor clinicians, providing support and serving as a resource for any challenges they face in the field.
• Participate in on-call rotation to ensure continuous support and availability.
• Ensure excellent patient satisfaction, positive outcomes, and maintain top-tier ratings.
• Maintain organizational compliance with all legal, regulatory, and accreditation requirements.
• Manage conflict resolution and complaints to ensure effective problem-solving.
• Contribute to budget management for the Home Health department, ensuring financial objectives are met.
• Ensure staff are up-to-date on the latest clinical practices and standards through training and ongoing education.
• Foster a positive, supportive work environment through effective supervision, development opportunities, and team collaboration.
• Serve as interim administrator during the absence of the Administrator, demonstrating strong leadership in all areas of operations.
Qualifications
• At least two years of experience as a Registered Nurse, preferably in home health or hospice.
• Supervisory experience in healthcare is preferred.
• Strong knowledge of relevant local, state, and federal regulations.
• Current, unencumbered RN license in the state.
• CPR certification (Basic Life Support).
• Proficient in Microsoft Office Suite (Excel, Word, PowerPoint).
• Excellent verbal and written communication skills with the ability to manage multiple projects.
• Ability to meet deadlines and work independently.
• Valid state driver’s license and satisfactory motor vehicle record.
Physical Requirements
• Ability to perform CPR and clear obstructed airways.
• Ability to climb at least two flights of stairs.
• Ability to stand and/or walk for at least four hours at a time.
Reporting Relationships
• Reports to the Administrator of WC@ Home.
• Manages the Home Health Team.
