This is more than a role—it’s a mission. You’ll assess and respond to the emotional, social, and psychological needs of patients and families navigating terminal illness. You’ll be their steady guide through overwhelming moments, their advocate when needs arise, and their support through anticipatory grief and bereavement.

Be a lifeline. Identify and respond to signs of mental distress, abuse, or suicidal risk, offering vital crisis intervention and short-term counseling.

Tailor care with heart. Contribute to personalized care plans that honor cultural values, family dynamics, and each individual’s unique needs and developmental stage.

Bridge the gaps. Connect patients and families to essential community resources, breaking down barriers that limit access and support.

Honor every stage. Support transitions to different levels of care, ensuring dignity and continuity through detailed, compassionate communication with care teams.

Walk with them. Provide ongoing bereavement support and assess risks of complicated grief, offering healing presence long after the patient has passed.

Educate and empower. Help families understand and participate in the care process while offering training and resources for your care team through in-service collaboration.

WHAT YOU WILL NEED TO BE SUCCESSFUL IN THIS ROLE:
Minimum of year social work experience in a healthcare setting.
A Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or
A Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a Baccalaureate degree in psychology, sociology or other field related to social work and is supervised by an MSW.
Demonstrated excellence in verbal and written communication.
Must be able to meet deadlines, work under limited supervision.
Must be able to walk up at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours
Valid state driver’s license
Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers
Proof of motor vehicle insurance
