What are the responsibilities and job description for the Home Health PRN Licensed Practical Nurse (LPN) position at Westminster-Canterbury on Chesapeake Bay?
Westminster-Canterbury at Home is currently hiring for Home Health PRN Licensed Practical Nurse (LPN).
Provides skilled nursing care with the goal of restoring, maintaining and promoting optimal mental and physical health of patients through assessment, teaching, counseling and appropriate preventative and rehabilitative nursing measures.
Conducts an initial and on-going assessment of the patient’s physical, psychological and social needs. Performs an initial accurate and on-going assessment of the patient. Identifies reasonable, achievable discharge outcomes. Includes the patient’s family, significant other, when indicated, in the assessment of the patient’s needs.
- Completes and submits all necessary documentation in a timely manner. Ensures documentation meets reimbursement criteria and adheres to agency policies and guidelines. Demonstrates competency in OASIS data collection. Demonstrates competency in all aspects of software clinical package. Completes and submits weekly payroll time sheets meeting processing timeliness.
- Communicates pertinent information regarding patients’ care and needs. Communicates admission status needs to Home Care and other disciplines involved in the case. Communicates changes in condition/needs in a timely manner to appropriate agency personnel, physicians, patient’s family/caregiver, and outside resources as indicated. Communicates/collaborates with members of the health care team through case conferences to meet patient needs, when indicated. Demonstrates knowledge of community resources and utilizes them appropriately.
- Plans individualized patient care utilizing professional knowledge. Organizes and analyzes data collected to identify patient needs and develop a plan to meet those needs. Develops an appropriate care plan and establishes plan of care for HHA and provides supervision and required updates.
- Provides individualized patient care utilizing professional judgment. Demonstrates appropriate decision making in implementing the correct nursing intervention and notifying the physician when indicated. Demonstrates knowledge of and performs patient care procedures, according to established policies and procedures. Evaluates and revises the plan of care based on the reassessment of patient’s needs and the patient’s response to interventions.
- Maintains compliance and completes mandated training and in-services on a timely basis.
- Understands the concept of performance improvement as a process for improving patient care and participates in the agency plan. Identifies high risk, problem prone issues and the need to monitor for performance improvement. Identifies, documents and reports incidents, participates in focus review for improvement process.

WHAT YOU WILL NEED TO BE SUCCESSFUL:
- Licensed Practical Nurse, with at least one-year experience in acute clinical care or two years’ recent experience in home health.
- Holds an unencumbered state license as Licensed Practical Nurse
- Current CPR certification (Basic Life Support)
- Demonstrates good judgment and decision making in setting priorities.
- Exercises initiative and willingly provides assistance to peers and managers.
- Prioritizes and manages time in an efficient manner and meets productivity standards consistently.
- Maintains current licensure requirements and seeks new knowledge in order to improve nursing skills and expertise.
- Strong computer skills, including Microsoft and Google applications.
- Must be able to meet deadlines, work under limited supervision.

- Valid state driver’s license. Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers. Proof of motor vehicle insurance.
PHYSICAL REQUIREMENTS:
Ability to perform cardiopulmonary resuscitation and relief of an obstructed airway
Ability to climb stairs and to stand and/or walk for a period of time.
