Position Summary: Hospice LPN
As a Hospice Licensed Practical Nurse, you’ll work under the guidance of a Registered Nurse to deliver compassionate end-of-life care. You’ll play a vital role in ensuring comfort, dignity, and support for patients and their families, coordinating with an interdisciplinary team dedicated to patient-centered care.
Key Responsibilities:
- Assist the RN in implementing the interdisciplinary care plan, delivering hands-on patient care.
- Document patient care and progress accurately in clinical notes.
- Support specialized care tasks and educate families on self-care techniques.
- Maintain effective communication with the care team to provide cohesive, high-quality service.
Qualifications Needed:
- Licensing & Experience: Valid LPN license in Virginia with at least one year of experience in Hospice, Home Health, or Skilled Nursing.
- CPR Certification: Basic Life Support (BLS).
- Communication Skills: Strong verbal and written skills for effective interactions.
- Technical Proficiency: Familiarity with Office and Google Suite software.
- Driver's License & Insurance: Valid state license with a clean driving record.
