Job Overview:

As a key member of the Westminster Place Activities team, this role oversees activity specialists in healthcare and memory support. Responsibilities include assisting with the hiring process, providing supervision, conducting annual evaluations, and offering ongoing training. Training areas cover documentation, group leadership, activity calendar planning, job expectations, and regulatory compliance.

Reports To: Senior Director of Community Programs and Volunteer Services

Job Responsibilities:

  • Responsible for activity specialists working in healthcare and memory support including helping with the hiring process, supervision, annual evaluations, and providing ongoing training in areas such as documentation, group leadership, activity calendar planning, job expectations and regulations.
  • Assist in the development of healthcare activities policies and procedures.
  • Establishes goals for healthcare activity programming and sets objectives for reaching these goals. Reviews objectives with staff on a regular basis.
  • Provide calendar analysis of the program while considering the seven dimensions of wellness, weekend, evening, daily programming and age appropriate needs of residents served.
  • Provide group, chart, 1:1 binder and monthly tally audits on an ongoing basis.
  • Monthly responsibilities include, but are not limited to: fliers, monthly attendance update, update daily census, calendars (development, printing, distributing and emailing calendars to families), hire and produce check requests for entertainers, produce resident birthday list and invitations.
  • Attend Resident Council meetings, record the meeting, complete minutes, copy and distribute to residents.
  • Participate in CQI activities and program evaluations as needed.
  • Purchase supplies as necessary for programs and projects working within a budget set by supervisor.
  • Plan, develop, and lead various programs as needed.
  • Participates in healthcare care plan meetings, reviews care plans on residents for completeness and accuracy. Ensures that residents’ care plans support the whole person and reviews and modifies care plans on a quarterly basis and as needed.
  • Work with the Activity Team to complete a Two Touch Life Story assessment for residents in memory care, whereby an initial life story is completed upon admission and then reviewed/updated within 60 days of admission.
  • In conjunction with the Social Work Dept, be the team lead to facilitate the virtual or in-person care plan meetings to include family members, POAs and ID Team for memory care residents.
  • Ensure that the Activity Specialists are attending, updating care plans on a regular basis for all healthcare residents.
  • Collaborate with other departments and attend meetings as necessary.
  • Attend and participate in weekly community programs team meetings, meetings with supervisors, and other meetings as necessary.
  • Ongoing growth and development from participation in workshops, in-services, conferences, and interdisciplinary meetings. Complete all assigned Relias Learning training in a timely manner.
  • Responsible for attending and tracking 10 hours of continuing education in activities and 12 hours of dementia-related training per year as required by the Illinois Department of Public Health.
  • Responsible for activities related to volunteer resources: supervision, daily assignments, check-in and check-out, tracking of volunteer hours.
  • Act as a liaison between administration and the Woman’s Board.
  • Coordinate Honor flight activities for residents.
  • Responsible for day-to-day operation of the Greenhouse.
  • Coordinate tuning and repairs of pianos.
  • Coordinate day to day and quarterly maintenance of Aviaries.
  • Responsible for Audio Visual training, usage and repair.
  • Responsible for Room Reservations system.
  • Responsible for learning set-up requests, e-catering, room reservation system, Caremerge, and operation of audio-visual equipment.
  • Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and/or clinical documentation. Responsible to use computers and software as required by work environment in order to enhance communications, performance and knowledge.
  • Is aware of Presbyterian Homes Corporate Compliance policies including those having to do with confidentiality and security of information, and concern for the wellbeing of residents and patients. Demonstrates behavior and completes work in a manner consistent with these policies.
  • Participate in WP Manager on Duty Program.

Education: Bachelor’s degree in human services or related fields.

Experience: Three to five years’ experience in Human Services. (Preference given to

previous work with elderly).

  • Counseling skills, communication skills. Capable of independent judgment, initiative, and ingenuity.
  • Ability to chart and assess by government regulations.
  • Have a proactive personality with an emphasis on teamwork and flexibility.
  • Ability to work with older adults in groups and 1:1.
  • Ability to maintain confidentiality.
  • Genuine interest in working with older adults.
