Westminster Place is seeking a dedicated PRN MDS Coordinator for the Nursing Administration department. Join the team at Westminster Place, a Presbyterian Living community, an independent, non-profit senior living care organization offering full continuum of care from independent and assisted living to skilled nursing and memory care at four luxury retirement communities.
Job Responsibilities:
- Ensures the completion of the Resident Assessment Instrument (MDS) within federally mandated guidelines and timelines for each resident.
- Processes Triggers and RAPS for admission, 14-day, annual, and significant change MDS, evaluates and adds to resident Care Plan as appropriate for both Medicare Part A and non-Medicare residents.
- Determines, at readmission of a resident, whether a new MDS must be completed or the previous one is adequate, depending upon whether or not there has been a major, permanent change in the resident's condition.
- Notifies all involved disciplines of the following:
- date an admission MDS is due
- whether a new MDS must be initiated for a resident who has been readmitted and date it is due
- date a new MDS must be completed for a significant change in condition
- date an annual MDS is due
- date a quarterly MDS is due
- Ensures that a minimum of 15 months MDS information is present in resident's active chart.
- Maintains personal records to track admission, Medicare, quarterly, annual, readmission, and significant change reassessment MDS for each resident.
- Reviews and analyzes QI/QM data available through software programs and CMS sites.
- Provides Administrator, DON, staff of trends and outcomes of reviews.
- Ensures that a Care Plan Conference takes place for each resident within 30 days of admission and every 90 days thereafter.
- Maintains the Care Plan Conference calendar in the computer on common drive so that it can be accessed by the interdisciplinary staff
- Ensures that changes in the plan of care, including goals and approaches, are communicated to the unit staff.

Education:
- Graduation from an approved professional nursing (RN) program.
Qualifications:
- Active, current Illinois RN license to practice professional nursing.
- Wound care certification or willingness to complete certification within 1 year of starting job.
- Current CPR certification – Accredited by American Heart Association.
- Experience with MDS and Care Plans recommended
- Knowledge of Medicare regulations and MDS 3.0
