What are the responsibilities and job description for the Acute Targeted Case Manager position at WESTMORELAND CASEMANAGEMENT AND SUPPORTS, INC.?
A full-time non-exempt position with primary working hours of Monday through Friday 8:30am-4:30pm (37.5 hours/week). Guaranteed hourly rate increase at 12 months of service. Paid training provided.
An Acute Targeted Case Manager shall have one of the following groups of minimum qualifications:
- Master?s degree in social work, psychology, rehabilitation, activity therapies, counseling; OR
- Bachelor?s Degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science AND three years of direct experience in behavioral health, of which two years must be experience working with or providing services to adult individuals with serious mental illness or to children with serious emotional disturbance or at risk for same. Experience as a case manager is preferred, but not required; OR
- Bachelor?s degree in nursing and 3 years of direct mental health care experience.
An overview of the responsibilities of the Acute TCM position include:
- Responsible for assessing client functioning and developing comprehensive Targeted Case Management (TCM) plans for individuals transitioning from psychiatric facilities.
- Coordinates discharge planning with hospital liaisons, links clients to housing, benefits, and treatment services, and ensures intensive monitoring during the first 90 days post-discharge.
- Oversees all services received by clients, ensures needs are met through natural and community supports, attends interdisciplinary meetings, and maintains a current directory of resources.
- Participates in a rotating 24-hour crisis response system, provides immediate support, and ensures all alternatives to hospitalization are explored.
- Acts as a strong advocate for clients, removing barriers, resolving issues, and ensuring access to appropriate services and resources.
- Maintains accurate records, ensures regulatory compliance, prepares for audits, and demonstrates strong knowledge of case management principles and community resources.
