QUALIFICATIONS

High School Diploma or equivalent

Social Service Designee Certification
-
Degree in Social Work, Psychology, Sociology, Gerontology is preferred

Knowledge of social resources in the community as it pertains to discharge planning.

CLINICAL/ADMINISTRATIVE
-
Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements
-
Assess the psychosocial, mental and emotional needs of residents
-
Assist with discharge planning
-
Implement and update Resident Care Plan and Social History
-
Communicate needs and plan of care to resident, families, responsible parties and appropriate staff
-
Implement and maintain an ongoing Quality Assurance Performance Improvement (QAPI) as required
-
Meet with consultants as required
-
Works with individual facilities to assist with the discharge planning needs of residents.

Coordinates services through the Social Service Department to promote organization of the residents’ specific discharge plans.

Assesses the individual needs and develops a discharge plan appropriate for the resident.
-
Coordinates the residents’ transition to appropriate placement, with various providers and insurances plans.
-
Communicates needs and the plan of care to residents, families, responsible parties and appropriate staff, as applicable.

Develops relationships with local community resources that may provide assistance with discharge planning options.

Participates in meetings, in-services, and training as required.
-
Participates in resident care conferences and/or meetings to discuss discharge planning, resources, and process of return to the community.

Attends training sessions as assigned by facility management and consultants.

Complies with all company policies and procedures as well as local, State and Federal regulations
