Description:
We are seeking a dedicated LPN Admissions Coordinator who will be responsible for creating an amazing first impressions with customers and fostering exceptional transitions to the health care center from hospital, outside organization and/or home. This role serves as the quarterback for creating the resident/customer journey that is second to none within the marketplace. The ideal candidate is detail-oriented, highly organized, and has a passion for delivering exceptional customer service in a fast-paced environment. This position will report to the Director of Admissions and Marketing.
Key Responsibilities:
- Responsible for meeting with families and residents to complete admissions contracts and paperwork.
- Responsible for conducting room readiness checks to ensure standards/guidelines are met, specialized equipment set up and hospitality/welcome special touches in place. Integrate with the rest of the WHV team in terms of special areas of attention, resident/family expectations and touch points.
- Assist with nursing admissions upon arrival.
- Assist with the process of insurance prior authorization and re-authorization.
- Responsible for providing facility tours as needed and/or to serve as a back-up to the Director of Admissions and Marketing.
- Responsible for regularly visiting new admissions and reaching out to families to obtain experience feedback and presenting to leadership team/follow up if necessary.
- Responsible for generating and trending admission questionnaires.
- Responsible for launching home visit program to discharged WHV residents and regular touch base calls/letters/cards.
- Responsible for assisting with the planning and hosting of special events in collaboration with the admissions/leadership team.
- Responsible for contacting families/residents regarding bed holds and completing bed hold letters to the corresponding hospital.
- Responsible for completing transfer notices for residents that are transferred to hospitals.
- Responsible for data entry for accepted referrals when serving as a back-up to the Director of Admissions and Marketing.
- Responsible for creating, launching and managing a post-discharge resident experience program.
- Responsible for serving as a back-up to the Director of Admissions and Marketing for patient on-site visits and/or assessments.
