1:1 Nurse - LPN/RN
Assist in meeting the physical and healthcare needs of the student with special physical/medical needs during the school day. Promote and maintain the health and wellbeing of student in the classroom setting in order to enable them to take full advantage of available educational opportunities.
Part-time position could lead to Full-time position
Qualifications:
- Valid current license to practice as a LPN/RN in the State of Massachusetts
- Graduate from accredited practical nursing program
- 1-3 years’ experience in pediatrics, community health, education, or related field.
- Current CPR certification
Essential Duties:
To the extent of his/her generic and continuing education and experience, the licensed practical nurse shall assume responsibility for the following health services, under the direction of the school nurse, and director of Health Services
Nursing Responsibilities:
- Collaborate with school nurse/lead nurse and the parents to develop and implement individual health care plan for student assigned.
- Maintain an accurate health record on student. Document health related activities and treatments on daily logs and/or computer based health record program as appropriate.
- Document administration of medications, treatments, first aid and illness care provided to student Complete accident reports.
- Communicate with parents as necessary regarding the health status of student
- Administer medications and perform treatments as ordered by the primary physician in the classroom setting.
- Provide emergency care in cases of injury or sudden illness, followed by referral to the primary care physician or proper medical facility.
- Collaborate with school nurse to screen all new students in classroom to ensure compliance with immunization law.
- Collaborate with school nurse to recommend exclusion and monitoring of re-admission to schools for students with infectious or communicable diseases.
- Maintain medical equipment and supplies as appropriate in the classroom per student Individualized healthcare plan (IHCP)
- Collaborate/cooperate with community providers and state agencies to meet the health and social service needs of student.
- Be a resource to the student, parent, principal and teachers in matters pertaining to health.
- Identify health and safety hazards in the classroom and advise school Nurse, Director of school nursing and teacher(s).
- Demonstrate knowledge of, and the ability to administer medications appropriately, in keeping with school policy on administration of medication in school.
- Other duties as assigned by School Nurse, Nurse leader per students IHCP.
- Must be experience with tracheotomies and tube feedings
- Provide personal care, assistance and supervision, as needed, for 1:1 students including:
- Dressing and undressing
- Toileting, including diaper change
- Grooming
- Feeding
- Positioning
- Transfers: i.e. may include heavy lifting
- Hygiene
Student Specific Needs:
Student with seizures, hemiparesis
Hourly assessments of student's neurologic status- Mobility monitoring with left hemiparesis
- Administration of thirteen standing medications as well as several PRN medications when needed
- Institution of new treatment plans as devised by his medical providers in the outpatient setting
- Ability and knowledge to liaison with medical providers to provide seamless and appropriate home care as his situation evolves and changes.
- Recognition and evaluation of myoclonus with subsequent expertise in treatment with swiping of the vagal nerve stimulator twice and then administration of Valium orally if it continues; also needs to recognize when the myoclonus is continuing and an MD must be contacted
- Recognition and evaluation of seizure with subsequent expertise in treatment with placement of Alex in appropriate safe positioning, administration of "rescue" medication (Valtoco) intranasally or rectal Diastat
- Continual observation and assessment after PRN medications are given for medications as they can decrease respiratory drive
- Recognition of emergent situation requiring 911 and emergency personnel Experience required
Educational Responsibilities:
- Assist student with proper body mechanics related to academic work
- Assist student in using adaptive equipment
- Assist with maintenance of classroom supplies including cleaning and care of equipment
