Position Overview :
Wheeler Staffing Partners is seeking an experienced Adult Psych - Social Worker to join a leading regional healthcare system, the largest between Worcester and Boston. This 307-bed facility is committed to providing high-quality, comprehensive care and has been recognized on the 2019 America’s 100 Best Hospitals List by Healthgrades. The Adult Psych - Social Worker will facilitate care coordination across the continuum to support optimal patient health, resource utilization, and safe transitions of care, balancing patient needs and self-determination.
Key Responsibilities :
Conduct complex psychosocial assessments and interventions to aid timely patient throughput, safe discharge, and readmission prevention.
Assess patients for transition needs and identify those at risk for readmission.
Coordinate care plans, ensuring effective resource utilization and adherence to regulatory standards.
Oversee and implement transition plans, leading multidisciplinary patient care conferences and complex case reviews.
Address adoption, abuse, and neglect cases, including intervention and referral to appropriate agencies.
Communicate care plans with patients, families, physicians, and healthcare teams.
Ensure patient education supports post-acute needs and maintain timely, thorough documentation.
Collaborate with ancillary departments and participate in quality improvement initiatives.
Precept new staff and serve as a resource for the team.
Qualifications :
Education : Master’s degree in Social Work (MSW) required.
Experience : Acute hospital experience preferred.
Licensure / Certifications :
MSW required; must hold current, active licensure, certification, or registration to practice as required by state law or policy.
Preferred : Accredited Case Manager (ACM) certification.
Additional Requirements :
Successful candidates must pass a background check and drug screening.
