We are seeking a dedicated and compassionate Director of Patient Coordination to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive the highest quality of care by coordinating various aspects of their Homecare services. This position requires an understanding of medical terminology, excellent communication skills, and a commitment to patient advocacy.
- Responsibilities
- Serve as a liaison between patients, families, caregivers, managers and staff to address concerns and provide support.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.
- Utilize knowledge of anatomy and critical care to assist in patient assessments and care planning.
- Review “patients on hold” for Homecare services weekly, and those not serviced for 30 days, to ensure they are discharged and readmitted when service is restarted
- Ensure Bi-monthly courtesy calls / follow-ups are conducted to active patients regarding our services
- Ensure weekly revision of hospitalized patients report and ensure coordinating managers checks in weekly about discharge status.
- Address department related issues in a manner that is consistent with the policies of the agency.
- Addressing patient complaints such as nurse cancellation, incidents with unprofessionalism and sensitivity.

- Experience
- Previous experience in a healthcare setting is preferred, particularly in roles involving hospice care or home care.
- Experience as a Patient Coordinating Manager is required
- Familiarity with medical terminology is essential for effective communication within the team.
- Experience with documentation review processes is advantageous for ensuring compliance with healthcare regulations.
- A background in nursing or related fields is a plus but not mandatory.
- Strong organizational skills and attention to detail are crucial for managing multiple patient cases effectively.
- Ability to work collaboratively within a multidisciplinary team.
