White Horse Village (WHV), a premier, not-for-profit CCRC located in Newtown Square, PA is seeking a Manager of Care Coordinator to assume leadership for the coordination of care management/support services and to serve as a member of the health care leadership team.
Responsibilities include:
Planning, directing, supervising, and implementing the services offered in the Outpatient Department.
Oversight of the day-to-day operations in the clinic.
Supporting the nursing team members, as well as the physicians scheduled in the clinic. The Coordination of the residential community’s case management program in collaboration with the nursing and social services teams.
Supporting the home care agency team/program coordinator.
Providing education about health management and available resources to residents.
Participation in the planning of educational programs for team members
Serving on committees, included, but not limited to, training, care coordination and the assessment of residents.
Offer programs/presentations to residents related to health and wellness topics of interest.
Qualifications:
- Licensure as an LPN Required
- Strong assessment skills, leadership experience, ability to provide orientation, education to team members, including person-centered support.
- Understanding of care management and experience working with the older adult population preferred.
- Excellent communication skills, experience in leading trainings/public speaking preferred.
