Job Summary:
We are seeking an LCSW or LPCC to support the acute mental health needs of our patients. The successful candidate will maintain same-day/next-day access for evaluations and provide brief mental health interventions to improve patient stability in the community.
Key Responsibilities:
- Evaluate the mental health needs of patients experiencing an acute mental health episode
- Determine whether hospitalization is necessary, and if not, provide stabilizing outpatient mental health care
- Coordinate care for patients to address social barriers and establish an effective plan for their mental health care
- Serve as an internal resource for crisis evaluation and community supports available for patients with acute mental health needs
- Collaborate closely with the Behavioral Health Provider team, Primary Care Providers, Behavioral Health support staff, and psychiatric care providers
Requirements:
The ideal candidate will have a Master's degree in Social Work or Mental Health Counseling (LPCCs must have 3 years' experience in a psychiatric setting) and be a Licensed Clinical Social Worker or Licensed Professional Clinical Counselor.
